At Dr. Christine Green’s Medical Practice, we’re committed to making things easy for you, so below we’ve organized the information that you’ll need to know before your first visit.

When you schedule your initial appointment with our staff, you will be invited to our Patient Portal through Green Oaks Medical Patient Portal. New Patient Forms will be “assigned” to you as tasks within the portal. Please enroll in the Patient Portal and complete these forms as soon as possible. Our staff will happily assist you if you have questions. These forms are essential to complete prior to your first appointment. In addition, records from previous physicians, lab and test results, prior physician notes and records will all assist us in evaluating your health and planning any necessary treatment prior to your visit.

Using the patient portal to exchange information with our office is the most efficient way to communicate and manage your health records. In the event that you are unable to complete necessary forms using the Green Oaks Medical Patient Portal, all forms are also available for download in the Forms Tab. If you are unable to upload prior records, notes and labs using the portal system, please bring your records to our office on a flash drive for our office to keep.

Items to Bring to Your First Visit

If you have not uploaded prior records to your OpenMedNet Patient Portal, please scan individual documents to a portable flash drive that our office will store with your records. Come prepared to take notes during your visit. Many of our patients find it valuable to also bring someone to the appointment to listen and take notes for them. All patients are expected to keep their own notes about appointment details.

Your First Visit

Your first visit to Dr. Green will begin with introductions to our staff, and will include a thorough evaluation of your health and/or medical condition. We’ll address any immediate medical needs you may have and will work with you to establish a follow up plan of care as necessary.

New Patient Form Packet

Please fill out these forms and bring them with you when you walk in our office for your first consultation. To view and print our forms, you will need the Free Adobe Acrobat Reader® software. Download Adobe Acrobat.

Instructions: Download the forms by clicking on each of them. Once the form is opened, click on the printer icon from the tool bar to print the form. Fully complete and sign the forms and bring them to your next appointment.

We see patients by appointment only. You may schedule an appointment by calling our Office at 650-433-8925.

Payment Policy

New patients are required to pay at the time of scheduling. Phone calls requiring the Doctor to call are charged according to complexity.

Insurance

WE DO NOT BILL OR CONTRACT WITH ANY INSURANCE COMPANIES. At the end of your visit, you will be given a Superbill with ICD10 diagnosis codes and CPT procedure codes that you can submit to your health insurance company to attempt to obtain reimbursement for services rendered at out-of-plan rates. (EXCEPTION: MEDICARE PATIENTS will be given a receipt but not a superbill to prevent submission of any claims to Medicare, Medi-gap. or other Medicare supplemental insurance plans Medicare patients should fill out the Medicare Consent Form (link) which states that you acknowledge that you may not submit bills from Dr. Green to Medicare. We do not participate, nor are we providers in any insurance plan. Patients should never assume any level of reimbursement from your insurance for office visits or services. Please contact your individual carrier in order to ascertain reimbursement to you, by your particular Insurance provider plan. Depending on the physician or staff time involved, we reserve the right to bill for reports, letters or other documentation to support claims for reimbursement sent to your health insurance company.

Cancellation Policy

You must call Dr. Green’s office to make any cancellations. For returning patients cancellations made three or less business days before a scheduled appointment are considered “late cancellations.”. We require that all patients have a current credit card on file to use for billing “late cancellation” fees.

For new patients cancellation 10 business days before the visit will result in full refund. After that we can re-schedule but not refund. We accept Visa or MasterCard or personal check.
